Clinical Changes

  • Where possible all transactions, charting and forms will be electronic and paperless
  • You must bring a clean mask (or one will be provided for an additional cost of $2/mask)
  • A Phi COVID-19 Intake Form must be filled out before your first on-site returning appointment
  • Online 24 hour before appointment COVID-19 Screening Form required before each on-site visit
  • Onsite COVID-19 Screening, Consent and Liability Waiver signed in person at each visit
  • Staff and Client Restrooms will be designated
  • There will be two Reception areas
  • There will be two ‘waiting areas’ with only one chair in each to be only used if necessary
  • Water/tea/candy station will be off limits to clients. Water is available on request
  • There will be numerous hand sanitizing stations
  • Clients are asked to bring only what is necessary to their treatment
  • Clients are asked to take everything they brought with them when they leave
  • 6′ (2 meter) distancing is required in all common areas of Phi
  • Signage for distancing, hand hygiene, donning/doffing PPE, etc. is posted throughout Phi
  • Almost all non-essential items have been removed from Phi
  • HEPA Air Purifiers in Waiting/Reception Area
  • New Formal Return to Clinical Practice in Regards to COVID-19 Plan is in place (see link)
  • New Protocols
  • 24 hour Cancellation policy not enforced for COVID-19 related symptoms.​
